According to the information provided by the Indiana State Medical
Association, Dr. Dale Kete Cockrell held the position as medical
consultant for the ISMA's Physician Assistance Program for 15 years.
The ISMA announced on 2 February 2004 that Dr. Cockrell has been
replaced by Dr. Fred Frick as new PAP medical consultant. So even
though the exact date is not mentioned in the Assiciation's official
announcement, it is obvious that Dr. Cockrell left his position in the
last days of January 2004, or 1 February at the latest.
Th current listings for Dr. Cockrell in directories provide this as
his office contact information:

Please note that several physicians are listed as having 317-655-4600
as their office phone number and 8401 Harcourt Road as their address.
Both the address and the phone number belong to the St. Vincent Stress
Center in Indianapolis, so it is possible that Dr. Cockrell is
affiliated with that institution.

I NEED to know exact date Dr. Cockrell resigned or was fired by the PAP,
He is not working and has NOT at St.Vincents for at least 5 years.

Dear abster1,
I have done most extensive research in the past weeks, but with no
results other than those already presented.
To get the exact date may be information that the ISMA might be
reluctant to divulge to protect Dr. Cockrell's privacy. However, if
you have a legitimate claim against Dr. Cockrell, or a specific reason
why you might need this information (a legal action, perhaps) then the
ISMA might be willing to provide you with this information directly.
It is highly unlikely that they would divulge this information to a
"third-party" researcher. Alternatively, if you have a legitimate need
for this information for legal purposes, you might ask a lawyer to
assist you in demanding that the ISMA provide the exact date of
dismissal.
Unfortunately, researchers can only provide customers with information
that is available in the public domain. Employee records are (and
should be) kept private. The closest information to a date of
dismissal that is available to the public is that which I have
provided to you.
