I need to know the United States market size for Central Venous
Catheters and Dialysis Catheters.  The phrase "5 million CVCs are
placed each year" appears throughout the literature.  I assume that
the bulk of these are for acute, critical care use.  The ideal Answer
would include both unit and $$ data for 2004 and future years, broken
down as follows:

Total Central Venous Catheters
    Acute CVCs
    Chronic (Tunneled) CVCs
        Cuffed High-Flow Dialysis Catheters
        Cuffed Central Venous Catheters (Hickman/Broviac/Groshong, etc)
    Implanted Ports (Infuse-a-Port, LifePort, etc)
    Peripherally Inserted Central Catheters (PICCs)
