Given that I do not work for a company that provides a pension or post
retirement health insurance, what should I start doing now (20 years
before retirement) to ensure that I will have good post-retirement
health coverage at a reasonable cost?  How does Medicare/Medicade come
into play for someone like me?

Myself and my Wife will need to be covered.
I earn a good salary.
I'm 40.
I'm in good health.
I live in Virginia, USA.
