I'm looking for the name of a Super Nintendo video game. Here are the
few details I can remember:
I played it ~1991, so the release date is before.
It's a shoot-up game. Moving left/right/jump/etc.
The game consisted of "cities", where each entrance had a door (I
think some were locked).
The music was composed of several classical music pieces. One of them
was Bach's "Toccata and Fugue in Dm" (google for fuguedm.mid if you
want to listen to it).
One (or more) of the levels was the moon. I think it's #3 (where #2 is
the first level on the right, and #3 the next from the left).
The first level on the left was kind of dark (black background).
One of the level had bats or vampires (I think it had a purple background).

I only need the name of the game, not a link or any other info. Thanks.

It's a NES game, not SNES

Another tune I remember is Mozart's Turkish March (I think this is on
one of the first two levels)

Well, not any of the games in the Castlevania series I've seen. Then
again, maybe it's one that I have'nt seen. Any idea which one?

The only NES game to feature Toccata & Fugue in D Minor is Gyruss, a space shooter.

It's not Castlevania, but I'm not sure what it is. Dr. Chaos? Amagon?
8 Eyes? Guardian Legend? Strider?

None of these. Maybe the new clarification would help?
