Since you didn't specify a country to look at, I concentrated on the
US. I generally look at www.cdc.gov for health related statistics.
In the PDF, http://www.cdc.gov/nchs/data/series/sr_23/sr23_020.pdf, I
found a table on the percentage of women aged 15-44 who have been
surgically sterilized or whose partners have been surgically
sterilized (pg 21). In 1995, 40.9% were sterilized; tubal ligation
accounted for 25.8%, hysterectomy for 7.4% and vasectomy for 12.1%.
As for the cost of these, this probably depends on the area, the
insurance, and the doctor. One site,
http://adam.about.com/reports/000091.htm, has surgical sterilization
falling into the $1,000 to $2,500 range. Another website,
http://www.smartersex.org/contraception/surgical.asp, gives the same
range and notes that a reversal can cost as much as $10,000 (as well
as showing that a vasectomy is cheaper).
There was an article last month on a new method of sterilization
called hysteroscopic sterilization,

which says that it costs about $500 less for patients as well as
having other benefits over other forms of sterilization.
