Where can I buy on the web or in a store extra wide mens formal dress
slip on shoes with bows?  I need sizes 10 EE or 9 1/2 EEE, in a black
tie dress slip on with the little formal bow at the front.

Florsheim's is the caliber of store that will fit your needs.  I would
suggest going in for a fitting:

They have wide sizes in formal/tuxedo dress shoes.

If there is not one near you, I can find 2-3 more stores like this in
your area, if that would be a satisfactory answer.

I don't think this will help.  The Google question specifies formal
slip-ons with bows, and the Florsheim page you send does not show any
shoe with bows.

No, the page does not show shoes with bows, however, their inventory
of formal dress shoes in wide sizes is much larger than that, and it's
likely you might have to order them.  I'd call them for you, but
checking a store in Seattle won't do you any good.  Use the Store
Locator and call them.

Thanks, but those appear to be dress shoes, not formal shoes like one
would wear to a formal black tie dress code event.

OK, let me look locally for a Florsheims.  They closed many of their
stores in NYC when dress codes went more casual.
