I am looking for specific public United States Medicare statistics.
Unfortunately I do not have the time to dig them up though I believe
this data should be available in a number of places. One good source: 
cms.hhs.gov in the statistics section.

Here's what I'm looking for:

I need Medicare claims processed data for 3 specific states (CA, NY &
FL) for the last 4 years (Calender year 2000 through 2003).

I need Medicare (Part A & Part B/Non-MSP) data for 
1) the number of claims paid per year per state, and 
2) the total amount paid per year per state. 

I also need this data separated into its 3 main categories: Carrier,
Intermediary, and DME (durable medical equipment).

I have found the data for one specific year and it was organized like
this, though it did have all the states listed:
