I am Eli Levi, a master student of environment at Tel Aviv University.

I am doing a research about the urban environment. My research
question is:" The local government as an entity that puts the
environment at the front". I need to design a questionnaire that will
be sending to the policy making personal in the municipalities,
 personal who deals with many environmental problems
or issues. The questionnaire should reveal and exposed the attitudes
and the opinions of the policy making personal in the local
authorities towards environment.

 The questionnaire should be comprehensive with many interests that
charcterised the city, and built with multiple choice answers (closed
questions).

Hello Eli,


The National League of Cities in the US publishes an annual report on
"The State of America's Cities".  The most recent report for 2004 is
here:

This report contains an excellent survey instrument, which can be seen
in Appendix 2.  While it includes environmental questions, it is not
exclusively focused on the environment.  However, I think it could be
readily modified for the purposes of your research.

Let me know if this is helpful.  And if not, let us know a bit more
specifically what sort of survey instrument you're looking for.

Below is a muliple-choice survey on the theme "Local government as an
entity that puts the environment at the fore". As per your specification,
it is designed to reveal the attitudes and opinions of policy-making
personnel in local government who deal with environmental questions.




Introduction
------------

Thank you for taking the time to fill out this survey. In order to be
statistically useful, each survey must be answered in its entirety by
one person, and no person should complete more than one survey.

The survey is divided into three sections: Programs, Priorities, and
