i am seeking a list of freely available business productivity survey
reports.  the intent of these surveys/reports is to determine what are
the major concerns of business executives re; their company's
productivity.  stated another way, what productivity issues do they
believe are both the most challenging and most necessary to solve.  
the surveys/reports are targeted at an executive level audience.  the
surveys should be based on the following survey criteria:  originally
published within the past 12 months;  surveying commercial businesses;
 not public sector (i.e., govt) owned and/or operated; not biotech
sector;  annual revenues of approximately $100 million to $1 billion;
based in canada or the USA.

you may get sidetracked by soft "people" human resources issues (e.g.,
 change management,  employee or stakeholder buy-in;  compensation
plans;  employee incentive programs;  shorter/longer work weeks; 
work/life balance) -- we don't want that!
