Need official document translated from Nepalese to English

I have a document that I need to translate from handwritten Nepalese
to English that needs to be part of a US government application form
that I am working on.

I talked to a lawyer who said there are dozens of companies on the web
that will do the translation, and that the person doing the
translation just needs to include this bit of legal text and sign and
date it.  That will do for my needs.

Here is the bit of legal text (just for the record):

Certification of the translation

I, name of translator, herby, certify that I am fluent in both and the
Nepalese and English languages, and that I have translated to the best
of my ability, the attached (name or desc of doc) from the original
Nepalese, to English, and, not making myself responsible for its
contents, that the proceeding translation is true and complete.

I am hoping to email this 1 page document (a TIF file of a scan of the
document) to some company and get the translation with the above bit
of legal text (signed and dated) faxed back to me.  I am hoping is
doesn?t cost more then $25 to $50.

I am in a bit of a hurry, so any help would be greatly appreciated!
