I have a copy of "Henley's Formulas for Home and Workshop" --there are
a couple dozen recipes for toothpastes of various kinds, but none had
your ingredients. If buy a copy of the book, you could experiment till
you come up with a recipe you like, part Henley's, part your
ingredients.

Simple baking powder works well and doesn't taste bad, in fact, it
tastes rather like the stuff my dental hygenist uses. Of course, it
does not contain the other ingredients that you ask about.

I'm with Cynthis here: there are many recipes online and off line for
naturally home-made tooth paste. Your given ingredients might be
something that would produce a tooth paste only in factory-like
conditions. Your insistance upon these ingredients might prevent an
answer.
