When you are medically unfit to perform military duties.  Loss of a
limb, loss of eyesight, disease or chronic medical condition which
effects your health.
Sounds as though you want to get out before your enlistment is up. Why
not just fulfill the terms of you enlistment?

this pub lists the criteria for seperating military persons
paragraph 1-32 has general info for medical seperations

chapter 3 has more specific info for what exactly qualifies somebody
as fit/unfit for military duty

You are pretty lame to be looking for a way to defraud the govt. 
Obviously there is nothing wrong if you are looking for a way to get
medically discharged.  Tell you what just become overweight and get
out like that...so sad!
