?Major improvements to the delivery of services have taken a
significant step forward with the start of work on a new elective
orthopaedic centre. The creation of an orthopaedic and
musculo-skeletal centre at Chapel Allerton Hospital will provide a
central facility for the diagnosis, management and operative treatment
of adults with both upper and lower limb complaints.?
?The redesigned service will mean that from the end of this year
patient access, clinical outcomes and responsiveness to the needs of
individual patients will be much improved. The centre will
significantly increase elective orthopaedic capacity in the city, not
only through the provision of additional, dedicated theatre
capacity, but also through a redesign of the service.?
?The orthopaedic centre ? which is expected to become a national
model for elective orthopaedics ? forms part of a £26m investment in
the reconfiguration of acute services in Leeds that will lead to
improved quality, reliability and, in some cases, increased activity
levels.?
?It will provide two additional operating theatres (making a total
of four), a 16-place post-operative ward, two wards and an outpatient
area shared with rheumatology. Supporting accommodation will also be
provided, enabling the whole elective orthopaedic team to work
together in the same environment.?
?The elective centre has been designed to complement the
planned centre for orthopaedic trauma ? the emergency treatment
of broken bones ? at Leeds General Infirmary as well as two specialist
facilities for colorectal and breast cancer (at LGI) and upper
gastrointestinal and hepato-pancreatobiliary cancers (at St James?s).
Other reconfiguration improvements include the expansion of A&E
services and the creation of specialist centres for neurosciences and
day case facilities (LGI), opthalmology and medical assessment (St
James?s) and pain management (Seacroft).?
?The separation of elective and trauma orthopaedics eliminates the
element of unpredictability, ensuring clinical teams will be able
to undertake a greater volume of pre-planned work. The reorganisation
is crucial in terms of the Trust?s ability to respond to human
resource pressures across the speciality, enabling more effective and
efficient working patterns for consultant medical staff.?
?A number of measures have been introduced in the past 12 months that
have positively impacted on infection control across the Trust.
Surveillance has been stepped up, leading to the introduction of a
pilot project in orthopaedic patients undergoing prosthetic joint
replacement. The project served as a forerunner to a national
compulsory surveillance scheme for orthopaedic-related infections,
enabling the comparison of infection rates to be made between trusts.?

?The surgical success in Leeds has attracted the international
orthopaedic product company Biomet to invest in a £20,000-a-year
fellowship scheme, which sees orthopaedic specialists from around the
UK and Europe working in the department on six-month attachments.
Surgeons can come to Leeds to gain further experience of ankle
replacement surgery, working with consultants and sharing knowledge
and best practice.?

?Mr. Wright tabled a paper setting out the case for the recruitment of
two new and one replacement Consultant Anaesthetists. He explained
that the new posts were necessary to meet access targets for elective
Orthopaedics and to facilitate the opening of the Orthopaedic Centre
at Chapel Allerton Hospital in January 2005. Mr. Wright reminded the
Board of the difficulties associated with the recruitment of
Anaesthetists and stressed the need to commence the recruitment
process immediately. He also explained that a further business case
would be brought to the Board before Christmas concerned with the
expansion of the Centre in order to allow it to meet its full
potential to deliver increased Orthopaedic activity and meet access
targets.?

KNEES UP AT CASTLE HILL HOSPITAL
?Former hip and knee patients are staging a fundraising sponsored walk to raise
money for Orthopaedic research and training at Hull and East Yorkshire
Hospitals Trust. The one-mile walk will take place at Castle Hill
Hospital on June 22, with organisers hoping to raise even more than
the £25,000 they collected in 2001.
Led by a Piped Band the walk is undertaken by people who have had hip or knee
replacements or any other orthopaedic surgery, and their families.?

30 Extra Hip Replacements ? with Help from South Africa
?PATIENTS waiting for hip replacement operations at Blackpool Victoria
Hospital are to be offered surgery by a team of doctors from South Africa as
part of a series of measures to cut waiting times.
Thanks to a Government cash injection of almost £2m a clinical team will fly
from South Africa to the North West to perform 300 hip replacement
operations for a number of hospitals across the region.
The team will be based at the Southport and Formby District General Hospital
between June and September 2003. Thirty patients waiting for major joint
surgery at Blackpool Victoria will be given the opportunity to be treated by
the South African team."
Kevin Clarkson, the Trust?s Directorate Manager for Orthopaedics, said:
??Patients who will be given the choice of having their hip replacement
operation at Southport Hospital will be those who have been on the waiting
list the longest and who fit the medical criteria for the scheme.?
"This waiting list initiative complements a lot of other good work that is
going on in the orthopaedic directorate to reduce the time patients wait for
treatment. These however are only short-term measures and the Trust is
putting together a strategic development plan for the future expansion of
capacity within our local hospitals."
?The Trust has been working hard to reduce waiting times for orthopaedic
patients. In February 2002 patients were waiting up to 18 months for surgery.
The longest waiting time is now 12 months for patients who are fit for surgery
and some patients are treated sooner than this.?

New Orthopaedic Outpatient Clinic Opens following £750,000 Revamp
?THE Orthopaedic Outpatients Department at Blackpool Victoria Hospital
has reopened this week following a £750,000 upgrade. The department
has undergone a major transformation to provide modern facilities and
a much-improved environment for patients. The numbers of
consulting/examination rooms have increased from eight to 25 and are
much brighter and more spacious. The seating in the waiting area has
also been expanded. New facilities have been created including a play
area for children and state of- the-art x-ray and patient information
systems.?

?The Trust?s Orthopaedic Outreach team have developed a transitional
care scheme with a local nursing rehabilitation home to reduce
patients? length of stay in hospital following hip replacement.
We have also had an agreement with another NHS hospital, which has
been able to undertake additional hip replacements.
By the end of November 2003, nobody will be waiting more than 270 days
(nine months) for either procedure to be undertaken routinely and this
achieves the Government?s standard targets for waiting times.?

Developing a new nursing role in surgery
In November Sandra Flynn, Orthopaedic Nurse Practitioner, carried out
her first minor operation.
?A nurse at Chester for 24 years, Sandra qualified as an orthopaedic
specialist nurse in 1995 and four years ago became a nurse
practitioner. 12 months ago, she learnt that one or two nurses in
other hospitals were doing surgical work. Sandra discussed developing
the role of Nurse Practitioner in this way with Consultant Orthopaedic
Surgeon, Ian Harvey. He suggested she consider undertaking carpal
tunnel decompression ? a type of hand surgery. Sandra began by
observing him carry out this operation in theatre ? she was there to
look, learn and listen. They drew up a learning contract, including
the required competencies. Sandra also developed the practice
guidelines for this new role as there were no national ones yet for
nurses undertaking surgery.?

?The Trust has gained the services of Dr D Banks, consultant
anaesthetist, Dr M Barbores, consultant respiratory physician and Mr M
Waseem, consultant orthopaedic surgeon. We are looking forward to a
dynamic year in 2004-2005.?
Day Case Unit
?In 2003-2004 the Trust was successful in bidding for £2.1 million to
build a Day Case Unit. This will mean that the proportion of patients
staying overnight can be reduced from 40% to 30%. Evidence suggests
that most patients would prefer to recuperate at home. Where this is
not possible an overnight stay will continue to be an option. It is
anticipated that the Day Case Unit will reduce the waiting times for
day case surgery. The building of the unit will facilitate internal
changes that should reduce waiting times for orthopaedics and general
surgery.?

?MAST?s latest gift of high tech equipment to Macclesfield Hospital
was handed over to orthopaedic consultant Mohammed Waseem by Janet
Ward, co-ordinator of the hospital charity. Janet said the £26.000
donation to the orthopaedic department recognised the extra, valuable
work the department was now doing. In the picture Janet sits on a
mobile operating table, fitted with a new £4,000 Osi Schlein ultra
shoulder positioner. This gives the theatre team easy access to the
patient, as they lean against the positioner. Robert Unwin is pictured
with a new £11.000 fluid management system, used in keyhole joint
surgery, similar to the Queen?s recent knee injury. MAST also donated
two shoulder arthriscopes, one elbow arthriscope and one wrist
arthriscope, used in keyhole surgery. Dr Waseem said: ?This has been
yet another very generous donation from MAST. We are most
appreciative.?

?The team is also making a valuable contribution to the new Clinical
Decision Unit (CDU), which is already having a significant impact in
reducing admissions?
?This is leading to beds elsewhere being freed up, particularly in
orthopaedics, which has benefits for other patients needing operations
such as hip replacements.?

Trust Receives Extra Money to Boost Services
?East Sussex Hospitals NHS Trust has been allocated £330,000 from the
'Action On Scheme' run by the Department of Health. £230,000 will be
used at the Conquest Hospital to purchase more equipment to boost
capacity to undertake joint replacement surgery and allowing a third
operating theatre to be upgraded to undertake orthopaedic surgery.
This will result in an increase in the number of orthopaedic
operations performed helping to reduce waiting times for patients
requiring joint replacement surgery.?
