I am assisting a small home health care business in developing a growth plan. The owners are interested in foundation grants to expand the business. They are willing to work through a 501c3 to acquire the grant funding. The focus is on improving the well being of low to moderate income patients by developing care and support programs that are not strictly medical or health related. For example, they propose to create a program to increase the visitation and contact frequency of the patient?s family and friends. The program could also include patient care training for caregivers who are family and or friends. I need contact information on foundations that would be interested in a proposal of this kind AND that require a letter of inquiry to initiate the grant application process. | |
