Hello, I would like to know which bank, credit union, or other
retailer offers Visa or Mastercard Gift Cards (pre-paid spending
cards) that meet the following criteria:

1. No fees or very low purchase fees ( < $2 per $500 of card value)
2. Can be purchased online
3. Can be purchased with any Visa or Mastercard credit card
4. Do not require the purchaser to be a member of the organization in question
5. Are issued with a PIN number, ergo they can be used as a debit card
(no signature) rather than a credit card (signature required)

An example of the type of card in question can be found at

Note that, in this example, one cannot purchase online. Other cards
that can be purchased online (e.g.
http://www.nationalcity.com/personal/promotions/giftcard_holiday/default.asp)
have very high fees. My goal is to find a card like the CharterOne
card - which has no fee - but can be purchased online.
