PLease provide the definition and application of risk management
through "bow tie analysis".  An example of the appliction is referenced at:

This is not the "bow tie" associated with neck ties and clothing.

Do you want that specific application of Bow Tie Analysis or would any
good example be sufficient? For example, would an analysis for
aircraft failure causes and outcomes be an acceptable answer?
