I run a phpBB forum for Bulldogs and have created a form at
http://www.bullieforums.com/petalert.htm (The form is not live and
submit does not function yet).

I want the form at the address above to do the following:
When a user fills out the form and clicks submit, I want the form to
add the info to a database which will keep adding the entries until I
download them, which I would like to have as an excel CSV file. (I
want the entries to stay in the database even after downloading them).

I have fairly good knowledge of MS Frontpage (I use 2003) and work
with HTML, web pages and FTP on a daily basis.

I have both MySQL and Perl on my hosts server. I really need to get
this up and running - please help me out.
