My ears stick out a bit, and because of this I cannot sleep sideways
without my ears getting pinched (which is very painful when I wake up
and lift my head from the pillow). Where can I find a pillow that
won't pinch my ears? I imagine there must be something available with
a special ear cutout or recess.

Or how about this?

"Sealy Posturepedic's Snore Reduction pillow combats disruptive
snoring patterns by positioning the head and neck for maximum airflow.
The patented design accommodates back sleepers with a central
head-shaped contour, while side sleepers benefit from contours on
either side of the pillow and ear wells to minimize pressure and
heat."
