"www.daisy.redi.tk"  URL - is it a legitimate 3rd party identity
verification website- I gave credit card, phone number info to verify
my identity or is it a website  used for identity theft?

The site does not even indicate who is the responsible person behind
it. And it uses a free re-direction subdomain. Every single letter on
that website cries "Hello, I am a scam! My only purpose is to make you
reveal your personal data!"

I agree with Scriptor.  It's screaming scam.

1)  Do you think that the Page Name:  Hey :)  --would be on a
legitimate 3rd party identity verification website?

2)  Why is the page not legitimate "looking?"  Notice all the empty
space on the bottom half of the page.

3)  There are no links to Privacy Statements.  There are no Terms Of
Service, nothing....

4)  On my computer, the graphic is a broken link, and the right click
function is disabled.

5)  I find it suspicious that the search terms:

"DateMate 3.0" verification

...bring up ZERO results in Google. The page is either so new it has
not been indexed, or there is code that prohibits bots from crawling
the page.  Either choice is bad.

6) These search results don't look promising:

I found a WHOIS record on the redi.TK domain.  The following is who
the domain is registered to:

Domain: REDI.TK

City: None Given
State: 	Baden-W�rttemberg
Country: Germany

However, that's not really where it stops.  If you go to the root
domain at www.redi.tk, you will see that the domain is setup to offer
free subdomains to registered users.  This means that anyone who signs
up for their services can obtain a URL just like the one you found. 
Because of this practice, it would make it almost impossible to find
out who the owner of that subdomain is without talking to the actual
owner of the REDI.TK domain, who I posted above.

I agree with all of the other advice that the commenters have left,
but the best advice I can give is to call and have the credit card you
entered cancelled.  Let them know what happened and you will need a
new card sent to you.

I went and lookedthe site, and found that beides from looking very
amatuerish it is collecting information using a formail script. This
is not a way to be collecting personal information - especially credit
card information.

After looking at the source code for the page, I see that the formail
script is being run from another URL - http://www.tonykeyz.com. After
visiting that site, I see that it is run by a DJ that is 20 years old.
He could be the operator of the info harvesting datemate site, or his
tonykeyz site may be running an outdated verison of formail that
allows an outside domain to access the data that it is processing.

I would definately contact your credit card company as others have
suggested and cancel your card. Good luck in the future and be
careful.
