1}An exportable list of the 5 digit zip codes

2}Seperate from the list a map that has the contiguous boundaries with
the 5 digit zip codes displayed inside {e.g. phone book maps}

There is no map showing the boundaries of the USPS Zip Codes.  The
reason is that that they're not polygons, and are difficult to map. 
Furthermore, they change periodically.  Please see
http://www.census.gov/geo/www/tiger/tigermap.html#ZIP for info.
