Hi there!

The Ohio Department of Taxation offers all state payroll withholding
information, forms and instructions online:

Employer Withholding Taxes
http://www.state.oh.us/tax/business_taxes_employer.html

...as well as municipal (city) tax forms and instructions:

Municipal Income Tax Forms & Information
http://www.state.oh.us/tax/municipality.html

You can find a comprehensive chart with the most recent Federal FICA
(Social Security and Medicare) withholding percentages here (these are
the same from state to state):

Payroll Tax Rates
http://www.jbartram.com/html/payroll_tax_rates.html

Federal payroll withholding information is found here:

Publication 505
Tax Withholding and Estimated Tax
http://www.irs.gov/formspubs/display/0,,i1%3D50%26genericId%3D11636,00.html

(For a quick version, the University of Texas at Austin offers a chart
explaining how to determine Federal income tax withholding here:

Payroll services
http://www.utexas.edu/payroll/withholdingtable.html )

I hope this helps!
