I'm looking for stretch cotton lycra (like 90% or more cotton, 10% or
less lycra), no poly or nylon, 2 way stretch in solid colors. I'm
looking to buy by the bolt wholesale for around $2 - $6 per yard and
am looking for several colors, especially pastels (light blue, dark
blue, light green, dark green, black, brown, burgundy, red, yellow,
orange, cream/off white, light grey, dark grey.) I need the fabric to
come from one source, not a few colors from this company and a few
colors from that company, etc.

Shpetim Zero in LA is a fabric sourcing agent who has worked with high
profile clients such as Christian Dior.  He has confirmed to me he has
one or more sources to order the fabric you're looking for:

I will look into this for you.  In the meantime, can you tell me what
you mean by "bolt?"  There isn't a consistent definition for this. 
How many yards are you looking to purchase at any one time?

I just mean that I want to buy it on the roll. Anywhere from 20 - 50
yards on a roll is good. Not flat-folded.

Thanks for the clarification.

This contact I've given you would be able to get swatches mailed to
you, so you can actually feel and see the fabric.  There's of course
no way for you to tell what a fabric feels like by ordering online,
and the only difference would be placing the order via phone vs.
online.  This wouldn't be an option for you?

The best thing I can find online is the following:

They seem to fit your criteria for fabric and price.  However, because
the yardage is different from roll to roll, they can only sell "by the
yard".  Their "by the yard" price is wholesale-level pricing.  Will
this work for you?

Enter "cotton lycra" (without quotes) in the left side search box to
find all fabrics fitting cotton/lycra criteria.

Also, I have confirmed this is indeed "2-way" stretch fabric (not 4-way).

Almost all of their stretch fabrics are rayon blends and are
close-outs (not re-orderable). :(  I need cotton/lycra blends in the
colors I mentioned above.

The closest thing I've found is Assaltex -- www.assaltex.com.

The representative I spoke to said you can order everything online;
however, they will not show colors online -- echoing the sentiment of
other companies: monitors can display colors differently than what's
in real life.  They don't want to be responsible for sending you a
fabric that you thought looked a certain way.

If this won't do it for you, please let me know.
