The top of the letter is titled "State of Florida, Department of
Health". The DOH & Osceola Regional Medical Center are the Petitioner,
and Dr. Kenneth Byerly is the Respondent.  The Case Number is
2003-13535, signed 1/10/05 by John O. Agwunobi, MD, Secretary,
Department of Health (Florida). Katharine B. Heyward, Assistant
General Counsel, DOH, Florida Bar #0104817.  This was involving care
that a patient received while in the emergency room at Osceola
Regional Medical Center in Kissimmee, Florida 34741.
