The Praxis series of assessments are administered by the Educational
Testing Service (ETS). Instructions are provide at the ETS website on
how to order score reports for tests taken over the last 10 years.

?MEET TESTING REQUIREMENTS: Except for vocational teachers*, all
individuals applying for a teaching certificate in Rhode Island must
pass the appropriate test(s) as noted below for the designated
teaching certificates. Test results must accompany the application.
Educational Testing Service (ETS) administers these examinations
through the PRAXIS Series of assessments. ETS can be contacted by
calling (609) 771-7395 or on line at www.ets.org. Registration
booklets can also be secured at most institutions of higher education
with teacher preparation programs.?

?Ordering Additional Score Reports
You may request that ETS send your scores earned on tests taken over
the last 10 years to additional institutions and agencies. Your
additional score report will indicate only the highest score earned on
each test and, for selected tests taken since September 1998, whether
you received the ETS Recognition of Excellence. The fee per score
recipient is $35. You will automatically receive a copy of your score
report confirming it was sent.

You can order an Additional Score Report online with a credit card. Go
to Online Ordering and login or sign-up for an account. Once you have
logged in, select ?Place New Order? and then choose ?Praxis Service.?
Follow the online instructions to complete your order.
...by mail
Download and complete the Additional Score Report Request Form , or
submit your request in writing to ETS. Include your name, address,
telephone number, candidate ID number (if known), Social Security
number, date of birth, designated score recipient(s), test date, and
the correct fee. Mail your request to: ETS-The Praxis Series, P.O. Box
6052, Princeton, NJ 08541-6052. ETS will process your score report
within 10 working days after receiving your request.

You can fax your request to 1-609-530-0581 or 1-609-771-7906. Be sure
to include all of the information above along with your credit card
information. ETS will process your score report within 10 working days
after receiving your request.
...by phone
You can also call our toll-free number, 1-800-772-9476, and pay by
credit card to request an additional score report. A customer service
representative will take your information, and your additional score
report will be issued within 10 working days of your call.?

will honor telephone or faxed request to send your scores to recipient
(telegram requests will not be honored). Your request will be
processed in approximately ten working days after receipt. If you
request that a copy of your score report be sent to a designated score
recipient, you will automatically receive a copy of your score report
confirming that your scores were sent as requested. Scores for a
specific test will be sent to a recipient only if that recipient is
eligible to receive those scores.?

This led me to the Rhode Island Department of Elementary and Secondary
Education website. I clicked on ?Teachers? in the menu at the top of
the page. I then clicked on ?Teacher Certification? in the menu on
