I'm looking for a simple list of the 50-100 largest health plans in
the US. I found this:

signficantly more information than I need. I'm hoping that Forbes,
Fortune, Business Week, or a trade publication prints an annual list.
The only info I need is: name of company, geographic area covered, and
# of members. 2004 data would be best but 2003 or 2002 will be ok too.

Larger lists all seem to be commercial, complex, and quite costly.
