I am looking for a pre-paid, store valued, gifts card with a pin
number.  The card can be
either (1)ATM,(2)visa, (3)mastercard, or (4)any other card that can be
used at an atm.  I must be able to "PURCHASE ONLINE" with either a
debit card or credit card. I am aware of Intrust bank as well as the
banks listed on visa's and mastercard gift card website.  I would like
the purchase fee to be 1% or less of the gift card value.  Example 
$500 card/$4.95 activation

It sounds like you are looking for a gift card/debit card/ATMcard all
rolled into one card.  If I may suggest that you take a really hard
look at a Debit Credit Card.  By choosing a debit credit card you are
going to be able to use it for all of the things you mentioned in your
question.  Give it as a gift card and let the consumer choose where
they would like to spend the gift - don't make them go to a certain
store to shop.  Give it as a stored value card - then after the amount
has been exhausted if you chose - you may add more funds.  This is
good for the college student that has a great many needs.  Use it at
any ATM as a ATM card or Bank card - it will come with a pin security
ID that you may chose.  A good place to find many choices - is a
website online that I use for my childrens Debit Credit Cards when
they are away at college.

number of cards to choose from with very competitive purchase fees. 
When they need something at school I can make sure they get it on
time.

Your best bet will be webcertificate.com. You can buy them online,
they are mastercard. I have bought and used them several times over.
The good thing about them, is that you can alway refill a particular
card upto 4 times.

You will get a card number, expiration date, pin, etc. You can buy
them using credit card and debit cards and you can buy and send them
to someone via email.

Do not get me wrong, I do not work for them, nor are mine doing a PR
for them. I am just telling you my personal experience from using
their product.
