I won't point you to Tom Smart's book but have you seen "Held Captive"
by two reporters, Jeanne MacIntosh and Maggie Haberman, written within
6 weeks of Elizabeth's return to her family, and "Under the Banner of
Heaven" by Jon Krakauer, which is about some Mormal cult activities.
I'm not citing them as sources for this reply, just for your interest
if you want more general information about the case and Mormonism. Ed
Smart's book would be worth looking at, as well as well as the TV
movie with the same name (Bringing Elizabeth Home) - the family did
cooperate with the producers of this movie.

1. Here are the charges as filed by the Salt Lake City District Attorney's Office:
Two counts each of aggravated burglary; 2 counts each of aggravated
kidnapping, and 2 counts each of aggravated sexual assault. These are
all 1st degree felonies. Wanda Barzee, Mitchell's wife, is charged
with the same six crimes. There are no names attached to the cases
online; one count may be for Elizabeth and the other for her cousin,
whom Mitchell also attempted to kidnap.

2,"Confronted in her bedroom by a knife-wielding man, 14-year-old
Elizabeth Smart was ordered to grab her shoes and march into the
foothills behind her Federal Heights home.
Hours later at a mountain campsite, prosecutors say, Brian David
Mitchell -- who had repeatedly threatened to kill Elizabeth and her
family -- allegedly "married" the terrified girl in a self-styled
polygamous ritual.
According to charging documents, Mitchell's wife, Wanda Eileen Barzee,
helped her husband consummate the ceremony by persuading Elizabeth to
remove her pajamas so Mitchell could sexually assault the girl.
During four months at the campsite, just a few miles from her home,
Elizabeth was often tethered with a cable and suffered additional
sexual attacks, charges allege." - From the Salt Lake Tribune, March
12, 2004. More details may be forthcoming if this case goes to trial.
Elizabeth's father is said to be interested in a plea bargain to get
this over and done with. http://tv.ksl.com/index.php?nid=84&sid=142531
3. Here's an article on Elizabeth and the Stockholm syndrome.

This paper, by a college student, also talks about the Stockholm
syndrome, Elizabeth Smart, and Patty Hearts. There are some citations
to other web sites at the bottom of the paper. An interesting read.

If you mean a public appearance by Elizabeth while she was kidnapped,
I haven't found anything.

5. See # 3 for Patty Hearst reference. Some differences: Patty Hearst
was 19, Elizabeth was 14 - one an adult, one a child. Elizabeth was
kidnapped for what might be considered religious reasons, Patty for
political. They both became more or less willing participants in
their abductions in that when given an opportunity to get away, they
did not. But I don't see a lot of similarities.
An interesting question to work on. I am posting this as a comment; if
