There is an excellent freeware tool available to generate any kind of
forms in php using a very simple web-based interface.
This software generates code that has to be stored on your server, so
please understand that I canīt provide the files but the software to
generate the code. Iīve been using the software for a lot of webpages
and Iīm sure you will be satisfied.

Please have a look at the PHP Formmail Generator at
( http://phpfmg.sourceforge.net/home.php ).
Itīs a tool that is used to

"create ready-to-use web forms in a flash. Once the form has been
generated, you have a full functional web form. Including error
checking of required fields, email address validation, credit card
number & expiry date checking, multiple attachments sending, and email
auto responding. "
(from the same source as above)

You will find a demo of the very simple but yet powerful web interface
of the generator there. To get copies of the sent form to two a-mail
addresses simply input the two e-mail addresses separated by a colon
in the field "Direct All Emails to:"

You will have to download the phpformmail generator code from the
website and follow the easy installation steps:

"Installation:
---------------------------
1. Get the source file and untar/unzip it as following step
Unix:
tar zxvf phpFormMailGenerator-php-x.xx.tar.gz
Windows:
use winrar or some other unzipping utility
2. Upload the unpacked directory to your web server
3. Make sure the permission of the directory has r+w+x ( read+write+execute ) Unix:
chmod -R 777 phpfmg
Windows NTFS (NT4/2000/XP):
. right click folder "phpfmg", select "properties"
. set allow Read+Write then "OK"
4. type the URL http://www.yourdomain.com/phpfmg/index.php to your browser

How to use Generator:
---------------------------
1. Define fields for your form.
2. What happens after click "Generate" button:
. Attempt to save your fields to a .ini file
. If the .ini saved, try to out put two files : yyyymmdd-xxxx.php,
yyyymmdd-xxxx.lib.php
. If all 3 files created, you will get a pop up window of your form
. Now you have a ready-to-use web form
3. Download two files yyyymmdd-xxxx.php & yyyymmdd-xxxx.lib.php
4. Rename file yyyymmdd-xxxx.php to what you want, such as order.php.
Don't rename file yyyymmdd-xxxx.lib.php.
5. Upload those two files to your web site.
6. Done! "
(from the same source as above)

As you see itīs really not complicated and there are just six well -
described steps to get your forms generated and running.

This is by far the best solution I can provide and I hope you will be satisfied.
If you should have any problem using the phpformmail generator please
post a request for clarification and I will be happy to give you
further assistance.
