I need a two-column table containing annual data since (hopefully)
1960 or earlier. The first column should be total reported revenue for
all the companies in the index. The second column should be the total
market capitalization of the securities in the index at the same point
in time (probably the end of the year).

If this data is difficult to get, some substitutions might be fine,
such as:
 - A similar index or group of stocks, such as the Dow Jones
Industrial Average

I will also need to know where you found the information.
And to clarify, there are three columns, because the first is the year, e.g.
  Year   Revenue   Market Cap

Since the companies comprising the S&P change over time do you want
today's S&P cos over time or do you want the S&P cos at that time??
