I am looking for some statistics from a trade publication or
university that would report customer loyalty due to errors.  More
specifically, I am looking for something that says:

For every 1 error 5% of your customers may leave you for a different company
For every 2 errors 20% will leave
For every 3 errors 45% will leave

If possible, it would be great if this could relate to the financial
services industry.
