I have done some extensive searching for you. BY FAR the best list of
actors who have played the Phantom character in Sir Andrew Lloyd
Webber's "Phantom of the Opera" is at:

That's quite the list! Now, while the venue is included, the actual
dates are not. However, there is another excellent page on this site
that lists dates for productions of this musical, along with complete
casting lists. If you wanted, you could cross-check with these lists
to get starting dates at least.
