I want to assign an adverse possession claim i have made on an island
that is connected to my property. do you have a form I could use?

Hi Ursa,
We need to know in what country or state you live.
Also your question seems unclear to me:  do you want to register a
claim of adverse possession?  You say "assign an adverse possession
claim I have made ..."
If the claim has been accepted/recorded, then the island is yours,
your real estate property, and can be sold, gifted, as you wish.
Or are you in the middle:  made the claim, but it is still not
recorded, and you want to assign it to someone else?

Here are a couple of sites on the subject.  As you probably know, the
key words for adverse possession are "hostile", "open" and
"exclusive", that is, the owner of the island must knowingly have put
up with your "open" use of the island in a manner that "excluded"
anyone else's access to and use of it.
You write that the island "is connected" to your property, which also
seems difficult to understand.
