What is the fastest way in general to delete 2 million rows out of a
10 million row table? Please include a full explanation of why this is
the fastest way.  Please think about different environments in which
this would run ex. OLTP, OLAP, etc.  What kind of index should be used
if any and why?  What should happen with existing indexes and
triggers?  Is there any way to not have the transaction logged?
