Under US Federal law, requests for information from military personnel
records must be in writing, signed, dated, and mailed to the office of
the National Archives and Records Administration in which the records
reside. The National Archives and Records Administration stores
records at several different facilities:
"Requests for military personnel records or information from them
cannot be accepted by e-mail at this time. The Privacy Act of 1974 (5
U.S.C. 552a) and Department of Defense directives require a written
request, signed and dated, to access information from military
personnel records. Our e-mail address below should only be used only
to request general information (hours of operations, procedures, and
forms) or to submit compliments, complaints or concerns."

Searching on [ navy records ], I found that the records you require
are stored at the National Personnel Records Center:
"The National Personnel Records Center, 9700 Page Boulevard, St.
Louis, Missouri 63132-5100 has custody of the official individual
personnel records of Navy commissioned officers separated after 1902
and Navy enlisted personnel separated after 1885. These records
include the full name of the serviceman, next of kin, date of
enlistment, date retired, units with which he served, and any medals
and awards to which the veteran was entitled, as well as other
details. Information from these records is available to the veteran,
or if deceased, to the next of kin. Contrary to rumors, the 1973 fire
did not destroy Navy or Marine Corps records."

To obtain the information you seek, you'll need to fill out Standard
Form 180:

Fill it out as completely as possible, sign it, date it and mail it

The form does call for the individual's service number. If you do not
have this number, you can usually get it by sending a letter to:

Be certain to include name, known dates of service, and ship on which
he served.
