I am interested in getting a list,say 25, of  most frequent surgeries  which 
- are elective  [i.e patient chooses to have surgery and are needed as
 immediate life saving related]
- exclude plastic/cosmetic surgeries
- which cost at least $4-5000 in US 
Would like to know  app. number and cost of each type performed
annually . [Example  Bariatric procedure  145000 annually at avergae
of $25000 each]

The second line should read ".. patient chooses to have surgery and IS
NOT needed as immmdediate life saving ..."
