Hello subtley1108-GA

A couple of things. First, with Chapter 13 and all the recent law
changes, it?s recommended a number of times in MI court documents that
you do not handle doing this yourself, but PLEASE get legal counsel.
If cost is an option, you can look into the Pro Bono Program.

?Many courts have local forms that either supplement those published
here or present variations on them. Anyone intending to file a
bankruptcy case or participate as a creditor or other party in
interest in a bankruptcy case should contact the clerk's office of the
particular bankruptcy court to determine whether there is a local form
that should be used. With respect to taking any action for which an
official form is prescribed, however, Bankruptcy Rule 9029(a)(1)
provides that courts may not "prohibit or limit the use of the
Official Forms."

the Local Court?s office and double checking.

?After notice and a hearing, the court has determined that the debtor
is entitled to a discharge pursuant to section 1328(b) of the
Bankruptcy Code without completing all of the payments under the
chapter 13 plan. Because this discharge is granted pursuant to the
hardship provisions of section 1328(b), it is referred to as a chapter
13 ?hardship discharge.? This order is not the dismissal of the case.?

The answers provided did not give me the actual (blank) forms that I
requested or the instuctions on how to prepare the forms. They were
links back to the Bankruptcy web site - no actual forms for my needs
are on the site.
