The simple truth is that there's a free program which will
do about everything this program seems to do, with the tiny
condition that you do a little reading to teach yourself how
to best use it.
If you look closely at the screenshots for Speedoptimizer,
you'll see pretty much the same options on all the dialog
windows:
Max MTU
Receive Window
Time To Live (TTL)
Session Keep Alive
MTU Auto Discovery
MTU Black Hole Detect
There's a lovely free program called DrTCP which will let
you change these settings manually, and an excellent tutorial
on BroadbandReports.com which will teach you precisely how to
use it. Once the settings have been optimized, there's never a
need to change them, so buying into a program which supposedly
offers constant optimization is sheer folly.
DrTCP uses a simple user interface to allow you to make changes
directly in little-known registry settinga which have to do with
optimizing your internet connection.

Read it thoroughly, following all the links, and you'll be
able to change the settings to suit your system in no time.
Once you've made changes to the settings, you can test the
results at DSLreports tweak testing page:

And you can test your download and upload speed on their
speed test(s) page:

I have cable, and my settings are as follows:
Tcp Receive Window: 49640
Window Scaling: No
Time Stamping: No
Selective Acks: No
Path MTU Discovery: Yes
Black Hole Detection: No
Max Duplicate Acks: 2
TTL (Time To Live): 128
MTU: 1500
I haven't changed these settings (or needed to) in over 2 years.
You might even try simply using these settings to start with,
and if you pass with flying colors on the tweak test page, and
show good speeds on at least one of the speed tests, then you're
good to go.
