for number 1. (the number of ENT surgery centers), I am looking for
the number of ambulatory surgery centers that do ENT surgeries (in
addition to other types of surgeries).  It would also be nice to know
how many of these centers do ENT only (if that is readily available).

Hold that thought! I personally know of a company in either Main or
New Hampshire that gathers and sells this type of data. They are
really excellent, I bought their data when I was with GE Medical
Systems. They telephone each record in their dtatabase, and the do
include Free Standing Surgical Centers. I remember their database has
the Diagnostic Imiganing equipment as well as contact names and phone
numbers.

The name is on the tip of my toung, I'll post back when I get rid of
this mental block. There is another competing company as well, but I
like the people and the data from the company in New Hampsire (or was
it Main?) a bit better.
