We currently use Excel to build survey grids and hand them out to employees. We would like to go completely electronic and this presents new difficulties. We know have to use Access. How do I set up the database tables and forms so the questions are listed on the right, the salespeople our employees are critiquing are listed along the top, and the responses for each question are filled in throughout the middle. We use a 1-5 system "1" being least likely, "5" being most likely. I need to be able to answer these kinds of questions: How many people responded "5" for question 1 for salesperson 1? How many people responded "4" for question 1 for salesperson 1? ETC Which salesperson has the highest average score for question 1? How do the salespeople compare with one another?
