I am looking for contact informtation for the medical journal titled
"Infectious Disease Practice". The editor-in-chief is Burke A. Cunha,
M.D. and the IDSN # is 0162-6493.  Having trouble locating a website
for the journal and/or publisher.  Can't even find a phone # yet.

NOT REQUIRED, but bonus points if you can also tell me the
circulation, % circulation by country, breakout of who subscribes to
the publication, publishing frequency, and any other tid bits you can
find about the journal.

It took some time and research, but I have finally found the street
address to contact "Infectious Disease Practice". Please let me
explain in detail how I proceeded, so you can be sure it's correct:

I first searched for the journal's name and Dr. Cunha's name, separate
as well as in all possible combinations. When I used the journal's
name with the Google Image Search, I received, much to my surprise, a
useful result: A cash card issued by MBC Publications Inc. for
Infectious Disease Practice, Burke A. Cunha MD [1]

The address for MBC Publications Inc. on the card reads 126 Brixton
Road, Garden City, NY 11530. However, I had to make sure that this
address is really correct and that MBC is still the publisher of the
journal. This, however, was confirmed by an Amazon subscription offer:

Knowing the location of the publisher, I searched the New York
Department of State database of registered companies [2]. And I was
lucky: Among the records of active corporations, I found the entry for
M B C Publications Inc. (note the empty spaces between the MBC
letters). The surprise is that we now even know what "MBC" stands for
- Marie B. Cunha:

This is the best contact information that is available through online
sources; unfortunately, I have not been able to trace a telephone
number, fax number or website. But at least, this is the correct
contact address both of the publisher and, most likely, also of the
editor-in-chief Burke A. Cunha himself.
