Is a doctor under legal obligation to provide a complete receipt for
services rendered, so that consumer can obtain insurance
reimbursement?

Did you request a receipt?  If they refused what reason did they
offer?  The issue of a receipt would seem to be normal businees
practice, especially when you would be submitting for reimbursement.
