Do Rainbow flip flops run small?  I wear a 5 1/2 and their flip flop  that I
want starts at size 6.

Most all Small Rainbow flip flops are sized from 5 1/2 - 6 1/2.  

The Extra Small is sized from 4 1/2 - 5 1/2.

Would you please clarify which style you're interested in and I'll try
to find out more information.

The sizing charts below are only a guideline, as each individual foot
varies in width, arch height, and bone structure. On initial fit, the
sandal should fit snuggly. This in turn results in a much better fit
after the sandal "breaks in." This is due to the foam forming to the
individual foot. Each pair of sandals may vary slightly, because they
are all handmade.
