Need five or more examples when an enterprise has made an erroneous
decision to spin off internal software development team into a
separate private (non-public) company. Erroneous means that the
enterprise has lost in the end (i.e. software quality suffered, or
enterprise has lost control of the core software product with some
negative results, or software company did not survive on it's own,
etc. that eventually led to substential financial losses to the
enterprise).

Ideal examples should involve banks or financial institution.

Shireesh, thank you for your comment. However, it consists of one-time
software errors that can happen regardless of who wrote the software -
a spin-off or an internal division.
