I am seeking information on best practices in (a) prevention of
diabetes among adults, (b) coordinating and improving health care to
ensure diabetic patients receive appropriate care and avoid costly
hospitalizations and disability, and (c) patient education / training
for diabetic patients.

Primarily interested in programs, initiatives, etc. developed by
health plans (HMOs, PPOs) and/or employers to improve the management
of diabetes and reduce the medical costs and other consequences of
adult diabetes.  Reports, case studies, briefings, PowerPoint
presentations.  Emphasis on web links that provide information in PDF
or PPT format.

Hi! Most of the programs in PDF, or PPT files I found are from
government and research organizations and not from employers and not
from HMOs.
