When it comes to which version of which uniform each team wears, how
does the decision-making happen on a given day for an MLB game?

Oh, what a good question! 

The decision about the ?uniform of the day? in Major League Baseball
differs from one club to the next. Typically a ball team has a uniform
or perhaps a particular jersey that they wear when they play at home
and one that is designated for games played away (Like the Yankees who
always wore pinstripes at home and gray on the road). When it comes to
a specialized uniform, in many ball clubs, including the Minnesota
Twins club for example, the uniform of the day, including the cap, is
the choice of the starting pitcher.

This custom, while not written in stone for everyone is fairly widespread:

Here?s a very good article on the subject about a player named Miguel
Cairo who was confused about what to do when he went from the Yanks,
where the issue is already worked out of a schedule, to the Mets,
where the equipment manager (of all people) is the one who decides the
uniform of the day. Both Yanks and Mets know about the informal
?starting pitcher decision? custom but neither of the clubs chooses to
observe it.
