I have a MS Word 2003 Form which I would like to post on my website
for opening. Once people open and fill it out I want two control
buttons which allow them to 1) Send the filled out form as an email
attachment to a fixed email address with a certain subject line (in my
perfect world this would include the results of a couple of the fields
they filled out) and 2) Print the form to their printer so that they
can sign the form and bring it with them to the appointment. I would
also prefer if no one can edit/change any other part of the form. I
have a
copy of the form posted at http://www.sharedserver.org/chepinfo/ if
you need to look at it.
