NOTE: I need an answer before 9-15-05

FYI: I am currently using Form Maker Pro http://www.form-maker.com/
and I believe slight modification will fix this issue. I have asked
the script author for help and he is not responding as of yet.

Basic Project Objective:

To create an online secure form that will allow our customers to
select from a list of 19 gifts (via checkboxes) as a bonus for
subscribing to our magazine.

Each of the 19 gifts are being provided by different companies.

When a gift is selected, the corresponding companies (that were
selected) need to be notified with the customers name and address and
our company gets their data sent to us for our records (a database
solution would be ideal but email will do).

NOTE: Recipients must be sent individual emails, not CC or BCC.
 

Detailed Description:

1. Customer pays for magazine subscription by Credit Card or Paypal online:

2. Customer fills out info

3. When ordering is complete customer gets redirected to promo (19 gifts) page
  -redirection only happens after payment is made
  -Promo page can not be accessed without payment

4. Customer selects some or all of the 19 gifts

5. Customer information (name/address, no credit or paypal info) is
auto filled from payment info.

6. Customer information is automatically sent to corresponding
(different) companies

7. Log with all information is kept for our records

8. Email to customer is sent confirming order of gifts

I can write this script for you. I just need to know:

The form only needs to know the customers name and address
You can make changes to the last page in payment system
What database system you are using
