I know of a service that can probably deliver a copy (by email or
fax), but they charge in the neighborhood of $40.

I'm not yet posting this information as an answer, since it's not
possible to confirm the availability of the article without actually
placing the order.

Infotrieve is an article delivery service with pretty extensive access
to scientific literature. When I searched for your article, they
quoted a price in the neighborhood of $40.
Hopefully, Infotrieve will be able to locate your article and either
email you a PDF or fax it over to you. Note that they offer a premium
"Rush" service, if that's of interest.
Let me know if Infortrieve doesn't work out for you, and we can
explore other options. In a few days time -- if I haven't heard
otherwise -- I'll post this information as a formal answer, so I can
collect the fee.
