I need Carotid Endarterectomy Surgery Statistics for San Francisco.
I would prefer raw data for all surgeries performed during last 5
years:
Surgeon Name,
Facility/Hospital
Surgery Date, 
Patient's Age,
Patient's Gender,
Severity of Stenosis (Severe vs. Moderate),
Result (I think 'Success' means there was no major stroke or death
during 1 month after surgery).
I would prefer data in Excel format or something that can be easily
converted to Excel.
