Hello chrishat !!

I found two sites that have the lists that you need, the utility that
these have will depend on the reason of your request.

I will guide you to find lists of Nursing Homes and Assisted Living
Facilities of each State of the United States, and to obtain relevant
data from each one.

If you search for Nursing Homes both sites work very similar, but one
of them need a free registration for get more info, and for complete
reports you must pay.

The first site is "Medicare.gov - The Official U.S. Government Site
for People with Medicare".
And the page to start your searching is "Medicare.gov - Nursing Home
Compare - Home"

"The primary purpose of this tool is to provide detailed information
about the performance of every Medicare and Medicaid certified nursing
home in the country. Important Information on Nursing Home Compare and
other resources, including the Guide to Choosing a Nursing Home, and
Nursing Home Checklist are also available to help you with your
nursing home choice."

Example of search:
To find a list of the Nursing Homes of Alabama you must click on the
link "Begin Nursing Home Search" you will guided to the first step of
four:

Here select the state of Alabama and click on the Next step Button.
The step 2 page appears:

Here you can choose for a Entire State search or refine your search by
County, Zip Code or Name. I choose Entire State Search and the site
shows you a warning about big searchs (more than 250 results). Read it
and click on next step button, the complete list of Nursing Homes of
Alabama appears sorted alphabetically:

Now you can select one or more Nursing Home to obtain more info of it.
Select for example the two firsts, and click the Next Step button at
the bottom of the page (there is a link to go to). A page with more
datailed data of the selected items is displayed. (I donīt give you
this page address because it results in an error, you must go to this
page by selecting at least one Nursing Home and clicking the Next Step
button).
Here you can choose to view additional information on one of the
following topics (for all of your selected nursing homes) by clicking
the appropriate button:
- About the Nursing Home: Provides information on nursing home
characteristics including the number of beds and type of ownership.

- Resident Characteristics: Provides data on resident characteristics
including the percent of residents with pressure sores, percent of
residents with urinary incontinence and more.

- Inspections Results: Including health deficiencies found during the
most recent State nursing home survey and from recent complaint
investigations.

- Nursing Staff: Provides information on the average number of hours
worked by the nursing staff per resident per day.

And for each Nursing Home you have a "View all infomation for this
nursing home" button to see all categories of information together of
the selected Nursing Home.

You can also download the databases from the"Download Database" page:
http://www.medicare.gov/Download/DownloadDB.asp

The databases are Ms Access 2000 files compressed in zip files, you
need an unzip utility to extract the files. In this page you have some
tips on how you can unzip files and links to download unzip utilities.
You must select wich database do you want to download and then click
the Download button.
Sometimes this kind of downloads doesnīt work properly, so I bring you
a direct link to download the list of Nursing Homes of the United
States:

You also need lists of Assisted Living Facilities.
I found a very similar service to get these lists by state, county,
city, zip code or Facility name at "CareScout" website.
The page where you can start your search is the "Assisted Living
Search" page:

The CareScout Assisted Living Database™ claims to be the first ever
comprehensive national resource available for information on over
38,000 assisted living facilities nationwide.

At this page select the state and if you want refine by filling the
other boxes (County, Zip Code, City and/or Name).
As example I choose only the state of Alabama.
You obtain 318 results sorted alphabetically shown of a ten results by
page.

In the list you can find Address and phone number to make a contact,
and also  drive directions to find the places, you must to input your
address zip code. You can see map situation of the facility but
before, you must register for free in the "Registration" page, you
only need to fill the Email, Zip Code and Password fields, the others
are optional:

CareScout also offers a service to find Nursing Homes, it works in the
same way of the Assisted living Facilities Search. The "Nursing Homes

The Online Survey Certification and Reporting (OSCAR) file. This is an
administrative database of the Health Care Financing Administration.
